Vitamin A deficiency is commonly seen in children of developing countries like India, Sri Lanka and Pakistan. It is almost always associated with protein-energy malnutrition. The ophthalmic manifestations of vitamin A deficiency disorders are collectively referred to as xerophthalmia by medical professionals. It not only includes the physical changes in the eye, but also the biophysical alterations. The WHO has classified xerophthalmia based on its severity. The manifestations are night blindness, conjunctival drying, bitot’s spot, corneal xerosis, corneal ulceration, corneal scar and changes in the fundus. Night blindness is the earliest symptom. The primary treatment for xerophthalmia is instillation of artificial tears which contain methyl cellulose. Oral administration of vitamin A is also recommended. The other associated conditions like protein-energy malnutrition and infections should be treated simultaneously. However the best approach to manage vitamin A deficiency is preventing the disease. Infants and children are given vitamin A supplements as per WHO recommendations. Fortification of food with vitamin A is a new approach. Educating parents to provide their children with vitamin A rich food like carrots and green leafy vegetables is an effective approach.
